Website questionnaires (before you create an account)
Our website offers short questionnaires that help you see which program could suit you. They may ask about your age range, goals and some health-related topics. Responses provided prior to account creation will not be used as part of your medical assessment. These answers are marketing information, are not reviewed by a clinician and are not added to any medical record.
Questionnaire answers are processed in your browser to show you the next question. They are not saved, and they are not sent to us or to our medical partners. They are not shared into a medical record unless you later create an account, complete the medical intake and consent to telehealth care, which starts a fresh assessment. Because these answers are health-related, we treat them as sensitive personal information.
The founding list
SlimBelle. is not seeing patients yet. If you join our founding list, we collect your email address, the state you live in, the program you’re interested in, your age range if you gave it in our questionnaire, your first name if you choose to share it, and the page you joined from. We don’t collect your questionnaire answers or any other health information when you join.
We use these details to email you when SlimBelle. opens in your state and to understand where demand is, so we can plan where to open first. We may share totals, such as how many people joined from each state, with prospective clinical and pharmacy partners. We don’t sell your details, and we don’t share your email address with partners without asking you first.
You can unsubscribe from any email we send, or ask us to delete your details, at any time.
How we use information
To provide and improve our services, communicate with you, process payments, meet legal obligations and, with your permission, send marketing. Protected health information handled by our medical partners is governed by our Notice of Privacy Practices.
Sharing
We share information with providers and pharmacies involved in your care, service providers who help us operate, and where required by law. We do not sell health information.
Your choices
You can access, correct or delete your information, and opt out of marketing, subject to legal and medical record-keeping requirements.
